How to Access Home Care

  1. Contact your local health authority or provincial disability services office. This is usually the starting point.
  2. Get a needs assessment. A care coordinator will evaluate what type and amount of support you need.
  3. Develop a care plan. Based on the assessment, you'll work with the coordinator to create a plan.
  4. Ask about self-directed options. If you prefer to hire your own workers, ask if self-directed funding is available.
  5. Apply for provincial disability income supportif you haven't already — programs like ODSP and AISH often include home care benefits. See our provincial programs guide.
